It is our goal to provide you with culturally based therapy services that meet the needs of you and your family. 

 

What to Expect:

At your intake appointment, your therapist will explore your current concerns, inquire about background history and establish the focus of therapy.

Before your first appointment:

Please arrive 10-15 minutes early to your first appointment in order fill out necessary paperwork.  Our new client forms can be conveniently completed online. We encourage you to fill out the following forms ahead of time and bring them to your appointment. You will need to print these forms. 

In order to best serve you and your family, we would like for you to learn a little more about our practice. Please read our policies and procedures here. These will be further discussed during your first appointment (intake).

What to bring to your first appointment: 

  1. Please bring your new client (intake forms)
  2. A photo ID
  3. Your Insurance Card 
  4. Discharge paperwork (if recently release from the hospital for mental health reasons) 

Our forms have been divided into an Adult and Youth packet, based on the age of the client(s).  

  • Parents, please complete the Youth Packet for children under 18 years old.
  • Clients 18 years and over, please complete the Adult Packet.

Insurance and Billing

Kindred Connections strives to offer affordable and accessible therapy services to residents in the west Dayton community. All fees are based on 50 minute sessions. Our therapists accept most insurances. During your intake appointment, we will confirm your mental health benefits. We will try to inform you of your benefits as soon as possible, as we understand the cost of insurance can be very expensive. 

If you have any questions regarding your insurance coverage, please be sure to contact your insurance company. Our staff will do our best to assist you as well. But remember, it is your responsibility to confirm your coverage with your insurance provider.
